3y.o toilet help

My son is 3 and has been relatively toilet trained for about 6 months.
He wees in the toilet without issue or without being told to.
Will not poo in the toilet. He will wear jocks all day and we then put night pants on for bed. About 20 minutes after they get put on he will come out saying hes done a poo and needs a change. I should mention he doesnt poo in his jocks either so I know hes holding it. He will ask for a nappy if he needs to do one.
This is every day. I am worried he is going to make himself sick.
We have tried EVERYTHING.
Rewards, bribery, jocks at night. I have toys as rewards in plain sight for when he goes. We have tried him watching YouTube on the toilet as a distraction, we sit in there with him, go in front of him. He goes to child care so he often will see other kids do it.
I am out of ideas. I've tried talking to him to explain he will make himself sick from holding it all day. He says he scared but wont tell me why.
Can anyone offer any help?

This was exactly like my son. In the end, I told him I wasn't going to be cleaning anymore of his poo nappies and made him clean it and get in the shower. He did this twice before he decided it was much easier to just poo in the toilet. No problems since lol
It will probably take something simple for your son to just switch since he is consciously holding on

When he does a wee in the toilet does he sit or stand?? I ask because I know my kids are afraid of falling in so my son was more than happy to pee standing up (on a little step) but wouldn't sit, so we found that a seat that makes the area in which they could potentially fall a lot smaller did the trick. It is quite normal at this age to be 'trained' for wee and not poo so try not to stress. If you're worried about him getting sick maybe give him a nappy once or twice during the day to go in and try again in a few months?
